1. The members agree to pay membership fee on a 3-month, 6-month, or yearly basis at the time of submitting this application. Renewal will be completed on or before expiration date.

2. Racquet Club’s rules which are posted at the club, shall be observed at all times.

3. Each member will receive a membership card. In case a card is lost, two hundred baht (200) will be charged for any new card issued.

4. Members may use all the facilities (including any of the badminton, squash, racquetball, tennis court, swimming pool, reading room, gym room and snooker room) without any additional charge.

5. Reservation can be made on same-day basis without any reservation fee, but next-day reservation require a fee.

6. Lockers are available on a first-come first-serve basis to all members without extra charge for daily usage. A nominal fee will be charged for any overnight usage.

7. In case of accident, members will be sent to the nearest hospital. The expenses incurred will be the responsibility of the members.

Note: When submitting this form, applicants must include two 2 x 3 cm. photographs of each member of the family and a copy of identity card or passport of the applicant.
